🧬 Introduction

 

Gemtesa is a prescription medication approved in 2020 for adults with overactive bladder (OAB)—marked by urinary urgency, frequency, and urge incontinence. It’s also indicated for men experiencing OAB while on medication for benign prostatic hyperplasia (BPH)


💊 Active Ingredient (Salt)

 

  • Generic Name: Vibegron

🧪 Mechanism of Action

 

Gemtesa works by relaxing the detrusor (bladder) muscle via β₃ receptor activation, increasing bladder capacity and reducing urinary urges associated with OAB


📋 Dosage & Administration

 

  • Recommended Dose: 75 mg tablet taken once daily, with or without food

  • Tablets may be swallowed whole with water or crushed and mixed with applesauce if swallowing is difficult .


⚠️ Side Effects

 

Common :

  • Headache

  • Urinary tract infection

  • Nasopharyngitis, nasal congestion, sore throat,/cold symptoms

  • Diarrhea and nausea

Less Common :

  • Dry mouth, pruritus, constipation

  • Minor liver enzyme increases

Serious:

  • Urinary retention—especially in patients with bladder outlet obstruction or concurrent anticholinergic therapy

  • Angioedema—potentially life-threatening; discontinue Gemtesa if it occurs

  • Monitor for urinary retention, particularly with bladder outlet issues or anticholinergics .

  • Avoid use in individuals with known hypersensitivity to vibegron or formulation components

  • Angioedema can occur as early as after the first dose—emergency treatment is required .

  • No significant blood pressure effects were observed at 12 weeks 


💡 Special Situations

 

  • Men on BPH medications: Effective for OAB symptoms (urgency, frequency, urge incontinence) .

  • Renal impairment: No dosing adjustments needed for mild to severe ; not recommended in ESRD or dialysis

  • Hepatic impairment: Avoid in severe hepatic disease

  • Pediatrics (<18): Safety and efficacy not established
