🧬 What is Obeticholic Acid?

Obeticholic Acid is a farnesoid X receptor (FXR) agonist. It helps reduce bile acid production in the liver and improves bile flow,

which is beneficial in certain chronic liver diseases.

🏥 Indications / Uses

  • Primary Biliary Cholangitis (PBC) – a chronic autoimmune liver disease that damages bile ducts

  • Used alone or in combination with ursodeoxycholic acid (UDCA) when UDCA is not effective enough

💊 Dosage & Administration

  • Starting dose: 5 mg once daily

  • May be increased to 10 mg once daily after 3 months if tolerated and if response is inadequate

  • Dose adjustments required for liver impairment

⚠️ Side Effects

Common:

  • Pruritus (itching)

  • Fatigue

  • Abdominal pain

  • Joint pain

Serious (rare):

  • Liver-related side effects (increased bilirubin, liver enzyme levels)

  • Decompensated liver disease (in higher-risk patients)

🚫 Contraindications

  • Complete biliary obstruction

  • Severe decompensated liver disease (Child-Pugh class B or C without adjustment)

🔄 Precautions

  • Monitor liver function tests (LFTs) regularly

  • Use with caution in patients with liver cirrhosis

  • Not recommended in pregnancy without doctor supervision

🔒 Storage

  • Store below 25°C

  • Protect from moisture and sunlight

Conclusion

Ocanat 5 (Obeticholic Acid) is used for treating Primary Biliary Cholangitis, especially when other treatments like UDCA are insufficient. It helps improve bile flow and liver function, but regular monitoring is essential due to potential liver-related side effects.
