Jaypirca (Pirtobrutinib) is an oral medication approved for the treatment of certain blood cancers, specifically mantle cell lymphoma (MCL) and chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL), in adults who have received prior treatments.

🧬 Composition & Classification

  • Generic Name: Pirtobrutinib

  • Brand Name: Jaypirca

  • Drug Class: Non-covalent (reversible) Bruton’s Tyrosine Kinase (BTK) inhibitor

  • Manufacturer: Eli Lilly and Company

Pirtobrutinib is a next-generation BTK inhibitor designed to overcome resistance to covalent BTK inhibitors, offering a treatment option for patients with relapsed or refractory disease.

📋 Indications

Jaypirca is indicated for the treatment of adult patients with:

  • Mantle Cell Lymphoma (MCL): Relapsed or refractory MCL after at least two lines of systemic therapy, including a BTK inhibitor.

  • Chronic Lymphocytic Leukemia (CLL) or Small Lymphocytic Lymphoma (SLL): CLL/SLL who have received at least two prior lines of therapy, including a BTK inhibitor and a BCL-2 inhibitor.

These indications are based on response rates observed in clinical trials, with ongoing studies to confirm clinical benefits.

💊 Dosage & Administration

  • Recommended Dosage: 200 mg orally once daily.

  • Dosage Forms: Available in 50 mg and 100 mg tablets.

  • Administration: Take with or without food. Swallow tablets whole; do not crush, chew, or split.

  • Treatment Duration: Continue until disease progression or unacceptable toxicity.

⚠️ Warnings & Precautions

  • Hematologic Toxicities: Monitor for anemia, thrombocytopenia, and neutropenia. Regular blood counts are recommended.

  • Infections: Increased risk of infections, including pneumonia and COVID-19. Monitor for signs of infection and manage promptly.

  • Bleeding Risks: Patients may experience bruising or bleeding. Use caution with concomitant medications that increase bleeding risk.

  • Secondary Malignancies: Cases of nonmelanoma skin cancers and other malignancies have been reported. Regular skin examinations are advised.

  • Drug Interactions: Avoid concomitant use with strong CYP3A inhibitors or inducers. Dose adjustments may be necessary if coadministration is unavoidable.

Conclusion

Jaypirca (Pirtobrutinib) offers a novel therapeutic option for patients with relapsed or refractory MCL and CLL/SLL, particularly those who have exhausted other treatments. Its unique mechanism as a reversible BTK inhibitor provides an alternative for patients with resistance to covalent BTK inhibitors. However, due to potential risks such as infections, bleeding, and secondary malignancies, careful patient selection and monitoring are essential. Patients should discuss the benefits and risks of Jaypirca with their healthcare provider to determine its suitability for their treatment plan.
