🧬 Introduction

  • Trelagliptin is a once-weekly oral DPP‑4 inhibitor used to treat type 2 diabetes mellitus. It enhances levels of incretin hormones GLP‑1 and GIP, boosting insulin secretion and reducing glucagon in a glucose-dependent manner

  • It was first approved in Japan (2015), and as of 2025 is launched in India under the brand Trelaglip by Zuventus Healthcare

✅ Indications

  • As an add‑on to diet, exercise, and metformin when glycemic targets aren’t met.

  • Approved as first‑line therapy if metformin is contraindicated or not tolerated

💊 Dosage & Administration

  • Standard adult dose: 100 mg orally once weekly

  • Renal impairment:

    • Moderate (eGFR 30–59): reduce to 50 mg weekly

    • Severe (eGFR <30 or dialysis): reduce to 25 mg weekly

  • Missed dose: take as soon as remembered; do not double dose

⚠️ Side Effects & Precautions

Common side effects: headache, nasopharyngitis, rash, elevated liver enzymes .
Serious concerns: hypoglycemia (especially if combined with sulfonylureas or insulin) and rare acute pancreatitis .
Contraindications: severe renal impairment (unless dose reduced), hypersensitivity to the drug .

📈 Pharmacokinetics

  • Selectivity & potency: high selectivity for DPP‑4 (IC₅₀ ~1.3 nmol/L), with a slow dissociation rate and long half-life (~54 hrs), supporting once-weekly dosing .

  • Steady-state reached by 4 weeks, with sustained enzyme inhibition (~77% after 7 days post-dose) .

🇮🇳 Availability & Brands in India

  • Marketed as Trelaglip by Zuventus, with additional suppliers like Cyno Pharmaceuticals (trelagliptin-100 mg)

  • Manufactured locally by companies such as Lifecare Neuro Products Ltd (Baddi, Himachal Pradesh; WHO‑GMP certified)

✅ Conclusion

Trelagliptin offers a convenient once-weekly dosing for type 2 diabetes, improving adherence and glycaemic stability. While generally safe, dosage must be adjusted in renal impairment, and caution is required when combined with insulin or sulfonylureas. It is now available in India, providing a new treatment option for diabetes management.
